The forms below are for reporting COVID-19 positive cases diagnosed by a health care professional,  a test performed by a testing site, or an approved test administered at home.  For those identified as a close contact (within 6 feet for 15 minutes in a 24-hour period) to a positive COVID-19 case, quarantine may be required during an outbreak situation.

The information contained in these forms is confidential and will be directed to your School Nurse. A nurse will contact you within 48 hours if necessary for verification of this information and to discuss DHEC/CDC and school district guidelines with you.
