• Email Sports Requests - If you have a suggestion for a sport that should be added to your school, please email Pupil Services Analyst Matt Campbell at mathew.campbell@kcsdschools.net.

All four pages must be completed. Students and parents must sign forms in all places required before eligibility is approved.

Students who are in a home school association or charter school will need to email the completed form below and proof of residency to Pupil Services Analyst Matt Campbell at mathew.campbell@kcsdschools.net.

Intent to Participate in Interscholastic Activities Home School Student

Schools will complete eligibility requirements if or when the student is selected for the team.  This may require providing a transcript to the school.

Students can only participate at the school at which they are zoned.

Students must have a physical and complete all forms before they are able to participate.  (See tab above for Physical Forms.)
